Naja, da geht das  PHB ja auch noch genauer drauf ein und sagt was es mit Phenomena oder Forces (Icy Hand in vulkanischer Umgebung).
Desweiteren sagt der Absatz davor eigentlich auch präzise "A conjurations uses your ability scores and defenses", also sehe ich es auch so, dass Konditionen den Angriff schwächen.

Auch denke ich, dass füpr einen attack ein Attackroll notwendig ist, was bei einer Falmig Sphere und ihrem Umgebungsschaden nicht der Fall ist. Also zählen auch Implementboni nicht zwangsweise dazu.
Beispielsweise Magic Wand sagt ganz klar +1 on attack and damage rolls when delivered through the wand.
