CRAWLING CHAOS
A writhing mass of tentacles and eyes that can phase in and out of reality.
AC 12, HP 65, ATK 3 tentacles (1d8 + 1 bludgeoning), S +5, D +3, C +3, I -2, W +2, Ch -1, LV 6
* Phase Shift. Can phase out of reality for 1 turn, becoming invulnerable and invisible.
* Eye Rays. Can fire a random eye ray (1d6) from its many eyes that can cause various effects, such as blindness or paralysis.
CHRONO HOUND
A hound-like creature made of metal and gears that leave trails of sparks as it moves.
AC 18, HP 38, ATK 1 Bite (1d8+3), S +3, D +5, C +2, I -3, W +0, Ch -2, LV 5
* Time Distortion: When the Chrono Hound is hit with a melee attack, it shifts time and avoid the attack.
SPINAL REAVER
A massive insectoid creature with many razor-sharp legs and a long tail stinger.
AC 15, HP 25, ATK 2 Leg Slash (1d8+4), 1 Tail Stinger (1d10+3), S +4, D +2, C +3, I -2, W -1, Ch -4, LV 5
* Leg Lock. If the Spinal Reaver hits a target with two or more Leg Slash attacks in the same turn, the target must make a Strength saving throw or be restrained until the end of their next turn.
* Venomous Sting. If the Spinal Reaver hits a target with its Tail Stinger, the target must make a Constitution saving throw or take an additional 1d6 poison damage and be poisoned for 1 minute.
RAT KING
A monstrous rat with a crown of bone and rotting flesh, surrounded by a swarm of smaller rats.
AC 12, HP 12, ATK 1 bite (1d4+1) and 1 swarm attack (1d6), S +1, D +2, C +1, I -4, W -2, Ch -1, LV 1
* Swarm of Rats. The rat king is accompanied by a swarm of smaller rats that attack as a group and move as one. * The swarm has its own initiative and uses the rat king's statistics for attacks.
* Disease Carrier. On a successful bite attack, the rat king can infect its target with a disease that causes the creature to take 1d4 poison damage per day until cured.
VENOMOUS VINE
A plant creature that appears as a tangled mass of vines, capable of ensnaring its prey with thorns.
AC 14, HP 20, ATK 1 thorny tendril (1d8), S +2, D +1, C +1, I -3, W +1, Ch -2, LV 2
* Ensnaring Vines. The vine can extend its thorny tendrils up to 20 ft. and ensnare a creature, requiring a Strength saving throw to break free. The creature takes piercing damage equal to the vine's strength each round it remains ensnared.
